Please read the following disclosure. Our goal is 100% customer satisfaction, so please ask any questions you may have to help us understand your needs and ensure a safe and enjoyable permanent jewelry placement.

PERMANENT JEWELRY IS A PROCESS THAT INVOLVES THE USE OF WELDING THE JEWELRY TOGETHER WHILE ON YOUR BODY. THE PROCEDURE IS PAINLESS AND IS PERFORMED WITH YOUR SAFETY IN MIND. THIS CONSENT FORM IS INTENDED TO INFORM YOU OF THE PROCEDURE AND TO OBTAIN YOUR INFORMED CONSENT FOR THE PROCEDURE.

Client Consent

I understand that although it is rare, an allergic reaction to jewelry can occur.

I understand that the welding procedure does have some risks, such as burns, skin irritation and other complications. My technicians have been properly trained and use a specialized welder that has built-in safety features to keep you safe during the process.

I have no known allergies to metals or materials commonly used in jewelry.

I have no medical condition that would impact my ability to receive permanent jewelry.

I have researched the risks and potential complications associated with receiving permanent jewelry placement and been informed by a representative of Rave Studio Salon LLC.

I agree to the jewelry care instructions recommended by the technician.

Care and Warranty

Our permanent jewelry comes with a 30-day warranty from the date of placement. The warranty covers defects in materials or workmanship under normal wear and proper care. The warranty does not cover damage caused by improper care, negligence, accidents, or unauthorized modifications. The warranty is non-transferable and only applies to the original recipient of the permanent jewelry. The warranty does not cover loss, theft, or damage due to force majeure events. Any alterations or repairs performed by unauthorized individuals or establishments will void the warranty.

We reserve the right to determine whether a warranty claim is valid and to provide appropriate remedies. We recommend periodic check-ups and inspections of the jewelry to ensure its integrity. Any necessary maintenance or adjustments required beyond the warranty period may be subject to additional charges.

How do I care for my permanent jewelry?

Polishing clothes are great! You can also clean your welded jewelry with warm water, a gentle toothbrush and dish soap. Sterling Silver will tarnish. It will need to be polished to maintain its shiny appearance. If you have hard water, work with harsh chemicals or are often in pools, this option is not recommended. I understand that although this jewelry is intended to be permanent, durable and long-lasting, the length of wear can vary depending on my physical activity and other factors.

How do I remove my jewelry?

I understand that there may be some instances where I will be required to remove my jewelry, including but not limited to medical procedures like MRI and surgery. I understand that the jewelry can easily be removed with scissors and re-attached by the jeweler at a later date.

What if my jewelry breaks?

Our welder will do a full inspection to ensure your jewelry is properly welded prior to leaving your appointment. Although our pieces are designed to be permanent, that unfortunately does not mean indestructible. If your welded jewelry is removed, stretches or comes off for any reason, keep the chain and book a re-weld appointment. If the chain breaks on the weld, we will repair it for free. Otherwise, there is a $15 re-weld fee.

What if my jewelry stretches?

Chains may stretch over time depending on the wearer's lifestyle. Chains can typically be resized for a $15 re-welding fee. If the chain is deemed stretched beyond repair, it may need to be replaced at the wearer's expense.

Photo & Video Release

I give the technician/jeweler permission to photograph my jewelry and procedure for their own marketing and promotional purposes. I hereby grant permission to Rave Studio Salon LLC, and its employees to take photographs or videos of me, my likeness, and my overall appearance during my visit/appointment.

I understand that the photographs and videos may be used for promotional and marketing purposes, including but not limited to social media, websites, print materials, and other forms of marketing. I understand that Rave Studio Salon LLC has the right to edit and use these photographs and videos as they see fit. I also understand that I will not receive any form of compensation or financial remuneration from the use of these photographs or videos.

I also understand that once the photographs or videos are used, Rave Studio Salon LLC has no control over the use of the images by third parties, who may also use the images for their own purposes. I release and discharge Rave Studio Salon LLC, its owners and employees, from any and all claims, demands, or causes of action that I may have against them arising out of or in connection with the use of the photographs or videos.

I hereby acknowledge that I have read and fully understand the terms of this release and that I have had the opportunity to ask any questions that I may have before signing.

By signing this release, I certify that I am at least 16 years of age. If under 16 years of age, a parent or legal guardian must sign this release. Any and all photo or video releases are voided if under the age of 18, unless consent is given by the parent or guardian.

I acknowledge that I understand the contents of this waiver and consent to permanent jewelry placement performed by a representative of Rave Studio Salon LLC. I have had the opportunity to ask any questions and have received satisfactory answers. I understand the risks and potential side effects associated with the permanent jewelry procedure. I understand that the results of the procedure are not guaranteed and may vary from person to person.

I am over the age of 16 and consent to the procedure. If I am under the age of 16, a parent or guardian must sign. I will not hold the technician/jeweler, salon, retail business, or employees liable for any issues not disclosed at the time of my service or any adverse effects from the permanent jewelry procedure. This agreement remains in effect for this procedure and any follow-up appointments.
